Your insurance plan may require a referral in which you will need to contact your Primary Care Physician. Please obtain this referral in advance of your appointment. It is your responsibility to understand your plan, you may have co-pays, co-insurance and deductibles that may need to be met. We will make every effort to work with your plan.
